This study has been approved by the Research Ethics Board at the University of Manitoba.
The following statements describe your rights and our responsibilities.
- I am the parent or legal guardian of the child on whom I am reporting.
- My agreement to participate includes consent for my child’s participation.
- Either of us may refuse to answer any question.
- We may end our participation at any time by closing the browser window.
- Our answers will securely stored.
- Our answers will only be shared with researchers on the Milestones team.
- Our participation does not affect our legal rights.
- Our participation does not relieve the researchers and the University of their legal and professional obligations.
